CNA Orientation

  1. Review the Intra-staff Observer Guide – see Documents below.
  2. Complete the Observer Guide Post Test – see Documents below.
  3. Review the Patient Care Standards Packet – See Documents below.
  4. Review the Hospital Standards Packet – See Documents below.
  5. Sign the Orientation Sign Off Form – See Documents below.
  6. Review and sign the Performance Standards Form – See Documents below.
  7. Please scan/email, fax or mail the following documents to the CMS office:
    1. Observer Guide Post Test
    2. Orientation Sign Off Form
    3. Performance Standards Form
