In November 2008, Franklin Square Hospital achieved Magnet Recognition from the American Nurses Credentialing Center (ANCC) for excellence in nursing services. Franklin Square joins the ranks of just five percent of U.S. Hospitals to have achieved this honor. Please note the following information specific for agency personnel planning to work shifts in this community hospital.

ID Badge: A FSH identification badge is required for all agency staff. The form to obtain an ID badge is in the Nursing Administration Office (open Mon – Fri 8AM – 4PM). Once you have an authorization form, please report to HR to have a photo taken. Then proceed to the Security Office (right next door to HR) to have your badge validated.

You are required to use your FSH ID badge to manually clock in and out. Franklin Square has a strict policy requiring badging in/out for all agency and staff. You may be penalized with a 2-hour loss of pay if you do not follow this policy requiring use of  badges to clock in/out at FSH. Any agency RN who does not clock in/out using their badge will be deducted 2 hours. In addition, please continue to use the CMS time slip obtaining an authorized signature at the end of your shift and fax to CMS as usual. You may wear your CMS badge under the FSH badge.

Please note: Nursing shifts in the ED, ICU, IMC & telemetry units begin 30 minutes earlier than most typical hospital shifts. Please confirm the exact start time of your shift with a CMS scheduling coordinator. FSH may cancel a shift with 1.5 hours notice rather than the standard 2 hours notice.

Please print the Post Tests and Sign-off forms. Complete in black ink and return to CMS by scan/email, fax or mail. ** Please note – you must show your math calculations on the worksheet in order to receive full credit for completing the pharmacology exam.
